In some cases, penoplasty is performed as an outpatient procedure under general anaesthetic, but in most cases a local anaesthetic under deep sedation will suffice.

The choice of the technique will be made between you, your anesthesiologist and your surgeon.

Your surgeon will adapt his operating strategy according to your particular case in order to obtain the best possible result.

For thickening penoplasty, the surgeon will perform lipofilling, reinjecting fat previously removed from the patient (inner thighs, abdomen, etc.) into the sheath of the penis.

The amount of fat removed depends on the patient’s reserves and also on the morphology of the penis. The grease is then treated by washing and centrifuging. It is then re-injected through small incisions using fine cannulas in the form of micro-particles between the skin of the penis sheath and the corpus cavernosum, in numerous independent routes to achieve a harmonious result.

Lipostructure is a definitive method, but the grafted fat cells are sensitive to variations in the patient’s weight, so weight stability is recommended. The head of the penis remains unchanged. At the end of the procedure, a light bandage is placed over the penis and the fat collection areas.

Depending on the clinical case, the operation can last from 60 to 90 minutes.

For lengthening penoplasty, the surgeon will perform 2 technical procedures, the first consisting of a skin advancement plasty. As a result, the incision results in a scar of variable shape, either an inverted Y located on the pubis and descending to the scrotum, or a Z-shaped scar.

Sometimes, depending on the morphology of the scrotum, a small additional incision is required at the root of the penis over the bursa. Next, the surgeon cuts the suspensory ligament of the penis, so that the part of the penis buried behind the pubis can move forward to achieve penile lengthening. The procedure lasts 30 minutes.

For combined penoplasty, the procedure lasts between 1h30 and 2h30.

The post-operative period is generally painless, rarely requiring simple analgesics. Any untimely erection will require adjustment of the preventive medical treatment.

For enlargement penoplasty, swelling may appear on the penis, which will fade in a few days, and bruising may also be present for two weeks.

The patient can resume social and professional activities after a few days. Sexual relations can be resumed after 3 weeks, and sport after 15 days, with the wearing of underwear.

Fat harvesting areas rarely restrict physical activity. Showers are permitted from the following day. No bandage is required.

We recommend wearing loose-fitting boxer shorts for the first few weeks, rather than usual underwear, which can cause inadequate compression. The sutures usually absorb in ten to fifteen days, by which time the wound has healed.

For lengthening penoplasty, the after-effects are painless. Swelling and bruising are common after lengthening. Careful local care will be performed daily for ten days or so, until the wound is completely healed.

A support panty must be worn over the dressings for a fortnight to avoid pulling on the scar. In principle, the sutures will disappear within ten to fifteen days, by which time the wound has healed. Showers will be reauthorized after complete healing. We recommend waiting 1 to 2 months before resuming activities such as motorcycling, cycling or horse riding.

For widening phalloplasty, a delay of 8 to 10 weeks is necessary to appreciate the final result. This is the time needed for all the swelling to subside and the tissues to regain their elasticity.

The penis now has its definitive volume. There may be a variable difference of up to 30% between the amount of fat reinjected and the amount actually taken up, but the practitioner will have taken this into account when assessing the volume of fat reinjected.

We can expect an increase in circumference of between 2 and 3 centimetres at rest. The erect penis is also enlarged, but to a lesser extent. A new distribution can also be observed on the erect penis, which is unhooded.

Scars gradually fade. After such an operation, there is no need to fear any change in erection, as the corpora cavernosa are not affected.

The result is definitive, but patients are advised to maintain a stable weight, or at least to avoid significant weight loss, even over the long term.

In fact, reinjected fat is sensitive to weight variations. If you lose weight, the result will be worse, but if you gain weight later, the result will be even better.

For lengthening penoplasty, the result can be judged after two weeks, as soon as the skin has healed. The penis is elongated by 3 to 6 centimetres at rest; when erect, the gain is non-existent or minimal.

Initially pink and inflammatory, scars gradually fade. Erections remain fully vigorous, but in some cases there may be a slight decrease of a few degrees in the verticality of the erection, which should not affect sexual relations.

The goal of this surgery is to bring an improvement and not to achieve perfection. If your wishes are realistic, the result should give you great satisfaction.
